If you could please complete the highlighted sections (sections 1, 3, 6, 7, 8, 9 and 10) and return this form to me at [email protected] I can then confirm your place on the above Short Course.
Please note, you must be over 14 years of age and resident in Scotland for the duration of the course. Additionally, EU nationals who have been granted ‘Pre-settled status’ OR ‘Settled status’ as part of the UK’s ‘EU Settlement scheme’, can you please provide proof of status by emailing your share code and DOB and the holders of ‘Resident in UK with Spouse Visa’ please provide photographic proof of the front and back of your visa or BRP card.
